Medical staff members help end problems at California hospital

Some California physicians say a recent settlement between a Ventura hospital and the Dept. of Justice underscores the importance of a self-governing medical staff.

Community Memorial Hospital in December 2007 agreed to pay $1.5 million to settle allegations that the facility gave improper gifts, loans and payments to some doctors in exchange for Medicare patient referrals. No doctors were named in the agreement, and the hospital did not admit any wrongdoing. Community Memorial administrators voluntarily disclosed the financial relationships, which took place under the hospital’s former leadership.

Finding one’s place (book excerpt: Intern: A Doctor’s Initiation)

In the fall I rotated through the geriatrics ward. One of the attending physicians was an irritating woman whose idea of the Socratic method was pimping you with really vague questions, then acting like she had already thought of whatever answers you gave and that you were only telling her what she already knew. The other attending was a throwback to “the days of the giants,” when pneumococcal pneumonia was diagnosed by injecting sputum into mice and antibiotics for urinary tract infections were tested on agar plates. One morning, one of my interns presented a case to him of an elderly man who had been hospitalized with fever and a cough producing green sputum. “He has pneumonia,” she proclaimed confidently. “Take a look at this chest X-ray.” She pulled up a digital image on a computer screen showing a distinct pneumonic streak. The senior physician waved it off. “First tell me about your lung exam,” he said.

Diabetes trial stops treatment arm in wake of death risk

Washington — The halting on Feb. 6 of an arm of the large type 2 diabetes trial, ACCORD, sent shock waves through the medical community. The lower-the-better conventional wisdom for blood glucose levels was questioned when it was found that those getting the most intensive glucose-lowering treatments were more likely to die.

But treatment strategies — keeping blood glucose, blood pressure and cholesterol levels in check — are likely to remain unchanged for the vast majority of the millions of people with type 2 diabetes. What might change is an overly aggressive pursuit of low blood glucose levels in patients with or at risk for cardiovascular disease.

Survey: Patients harmed by anemia drug policy

Washington — A new Medicare coverage policy on drug treatment for anemic cancer patients is hurting care, according to a survey of doctors released last month.

Ninety-one percent of oncologists and hematologists reported adverse patient events in the 12 weeks after the July 30 implementation of the national coverage determination on erythropoiesis-stimulating agents. The poll was sponsored by U.S. Oncology, which funds, develops, and helps manage 443 cancer centers in 39 states. It surveyed 307 physicians from Nov. 26, 2007, to Dec. 11, 2007. The organization limited the number of affiliated physicians involved in the poll to 20% of all respondents.

Parity, genetic privacy bills face snags

Washington — Bills to protect Americans from misuse of their genetic information and to prevent discrimination in mental health benefits attracted broad support last year but still face a tough road this year because of unresolved disputes.

Both the Genetic Information Nondiscrimination Act of 2007 and the House and Senate mental health parity bills have an array of co-sponsors and support. All of the measures, except the House mental parity measure, were approved by one chamber of Congress last year.

Alcohol hand rubs not enough to curb hospital infections

Making alcohol-based hand gels common and accessible in hospitals increases their use and cuts the amount of microbes on the hands of nursing staff. But it does not cut the rates of device-associated infections, illness caused by drug-resistant bacteria or cases of Clostridium difficile, says a study published in the January Infection Control & Hospital Epidemiology.

Authors and other experts say this situation is not because the products are ineffective. Rather, increases in their use need to be carried out in conjunction with other infection control strategies.
